Extending care to our communities through Medicaid Waivers

People who are generally utilizing the Community Care Service Program (CCSP) through Cherokee Angel Senior Care and Training Center are seniors and people who cannot live alone. These individuals do not require the level of care found in nursing homes, but they do require minimal assistance with activities of daily living. CCSP recipients have a monthly income of $400 to $1809 monthly.

Individuals served through the program must be eligible for Medicaid and meet the following requirements:

  • The person has health needs that can be managed in the community through the services offered by the program and within the established individual cost guidelines
  • The person has physical or cognitive impairments or limitations, making it difficult for them to perform normal daily living activities

How long does it take to be enrolled in the Community Care Service Program (CCSP)?

The process usually takes 2 to 4 months from the starting point to finish. However, clients can still move into the Personal Care Home (PCH) while their application is still in process. If the individual moves in the middle of the month, the due for room and board rate is pro-rated based on the number of days left in the same month.

COMMUNITY CARE SERVICE PROGRAM

Client using the CCSP Program through Cherokee Angel Senior Care are elderly who are live alone, and need assistance with activity of daily living (ADL). Although they meet the requirements for nursing home Medicaid, they do not need the level of care provide nursing homes.

To be eligible for the CCSP Program applicants must be eligible for Medicaid and meet the following requirements:

  • Have cognitive and or physical impairments/limitations which make it difficult to perform normal daily living activities and live independently
  • Have health care needs that can be met in the community with services offered by the program and within established individual cost guidelines.

What does this mean?

If an applicant has Medicaid (or qualifies for Medicaid), meets the above criteria, and has an income that is less than $1900 dollars per month, they may qualify for the Community Care Service Program (CCSP) and obtain residency in a licensed personal care home.

How much does it cost?

CCSP program recipients are responsible to pay the room and board of $666.00 per month directly to the Personal Care Home (PCH) where they live. The $666.00 is a rate established by the Georgia Department of Human Services, Division of Aging (01/01/21). Depending on their income level, they are also responsible to pay a cost share.

What is the Cost Share?

The Cost Share is an amount determine by DFAC and is used to offset the amount not paid by the State of Georgia under the CCSP for Alternative Living Services (ALS) also known as personal care home services. CCSP and DFCS determine the cost share.

How long does it take to be approved for CCSP?

It typically takes 3-4 months from start to finish for an applicant to be enrolled in CCSP. If applicant is deemed to be eligible for CCSP, they can move into a PCH while awaiting application completion and approval.

SERVICE OPTIONS USING RESOURCES IN A COMMUNITY ENVIRONMENT

SOURCE is available to the frail elderly and disabled who are eligible for or receiving SSI/Medicaid (Supplemental Security Income). People under age 65 are eligible provided that they have a disability and are receiving SSI/Medicaid. If you or your loved one is receiving SSI/Medicaid you may be eligible for the SOURCE Program.

How much does it cost?

Residents are required and responsible to pay the personal care home the room and board of $666.00 per month. The $666.00 is a rate established by the Georgia Department of Human Services, Division of Aging (01/01/21) out of their monthly SSI check of $783. The remaining $123.00 is kept by the resident for personal needs. Understand that the $666 pays for room and board which includes a bedroom, access to the rest of the home, 3 meals a day, snacks and bi monthly medical monitoring by a nurse.

SOURCE requires an assessment to determine eligibility. Clients must meet Intermediate Nursing Home level of Care in order to qualify. A memory deficit and or an obvious physical disability as well as a chronic medical condition are required in order for a person to be eligible for this program.

How long does it take to be approved for the SOURCE Program?

Provided that a prospective client is already enrolled on Georgia Medicaid it typically takes 2 months from start to finish for a client to be enrolled on ALS SOURCE Services. Clients can still move into the PCH while their application is in process.

ICWP

Indipendent care waiver services program

ICWP is available to adults 21 to 64 who have been determined disabled according to the Social Security Administration or the Department of Human Services and are financially and resource eligible. Also eligible are adults (21 – 64) who have a severe physical impairment and/or a traumatic brain injury that substantially limit one or more activities of daily living and require assistance of other individual(s). Although disabled, applicants must be cognitively alert and capable to participate in the development and management of their care plan initially and ongoing.

How much does it cost?
